PROSTHETIC TOOTH STRUCTURES WITH METAL BASE: INDIVIDUAL APPROACH

Abstract

Abstract. The aim of study is warning of the phenomena of impatience to the metal particles in the environment of the patient's mouth, caused by the presence of fixed metal constructions and electrochemical potentials. Object and methods. It was studied 18 patients who turned to the clinic prosthetic dentistry with the aim of making the aesthetic dental structures. We offered our method of express Diagnostics of metal for the selection of the basics of frame dentures. Widely in dental clinics tests for choosing metal is not held. It is well known the definition of electrical index, the results of which, prostheses are subject of destruction. But the electrical index measure after manufacture and cementation of dental structures, leading to the loss of the clinic and patients. In addition, patients gain a negative experience that is worsening the relationship doctor-patient.

A quick diagnosis which we recommend is made before the prosthesis of constructions. It is cutis (the application) standard. Not violated the integrity of the skin or mucous membranes. To do it, on the intact surface of the forearm, processed 70 ° alcohol, left is made of an alloy of the metal square area of 1 cm, which is the top cover cargoes with anything with plastic wrap and put on a band-aid, at a distance of 3-4 cm, a similar box with 4 layers of cheesecloth, of the penetrate of the saline solution. To cope with the 24-48 hours, and then carried out the assessment, namely:

-expressed hyperemia +,

-congestion, swelling, papules ++

-intense flushing, swelling, papules, and bubbles +++,

-the formation of the blister or else on the surface necrosis ++++.

Results and discussions. Using the express choice of metal for dental designs we selected materials for dentist and everyone who asked any help. We have 18 of the application of the cutis. 5 patients, with diabetes, has demonstrated different degrees of allergy to nickel-alloy Chemistry (electrochemical potentials were within the rules but were manifestations of hyperemia appearance, edema, subjective manifestations: heartburn, itching, etc.); were made of they dentures with alloys with CHA-basis test that was within the norm. Six patients of the mucus (internal mouth) tests with the purpose of refining the results obtained on the reaction, since the application tests of CHA and nickel chrome plating were outside the norm. After a mucus (internal mouth) tests, patients were asked to produce without the metal or contact one of the dental firms which produces dentures based on precious metals. For the seven patients whose tests on alloy KHS was within the norm, and the alloy of nickel were manifestations of allergies, was offered prosthetic silver alloy with Palladium, there were the defects of the teeth on side areas. We observed patients within a year. Measure the potentials of electrochemical that ranged from 60 to 80mv.

Conclusions. The method of express choice metal for dental prosthesis allows You to notify cases of occurrence of symptoms of impatience to the metal particles in the environment of the patient's mouth. Simple method for implementation does not injure any damage to the skin or the mucous membranes of the patient and allows the doctor or dentist to choose alloy metal individually.
