CHANGES OF THE HYGIENE AND PERIODONTAL INDEXES SOON AFTER THE MANAGEMENT OF CHRONIC HYPERTROPHYC GINGIVITIS IN THE ORTHODONTIC PATIENTS TREATED BY BRACKET TECHNIQUE

Abstract

Introduction. Scientific researches that investigated the influence of modern fixed arch orthodontic appliances on the periodontal tissues have proved the importance  of this problem. The prevalence of periodontal disorders in form of a catharal or hypertrophic gingivitis is around 55-62% by administration of the bracket technique.

The aim of investigation: to determine the efficiency of the therapy in treatment of chronic hypertrophic gingivitis by assessment of oral hygiene and periodontal tissues after three month of administration of the treatment in orthodontic patients handled by bracket system.

Materials and methods: we have examined and prescribed treatment to 126 patients from 16 to 35 years old that were affected by chronic hypertrophic gingivitis as a result of the fixed orthodontic therapy. The patients were divided into two groups- the main group and comparing group, 63 individuals were in each. The primary topical periodontal therapy was identically administer to the patients of both groups. On the other hand, the patients from the main group were prescribed with Cycloferone, Erbisole and Amizone.

Results. The patients noticed improvement on general well being and absence of halitosis after 3 month since the treatment as well as better gingival appearance and shape of the interdental papilla, decrease of gingival bleeding and sensitivity by tooth brushing. The gingival discomfort also disappeared. The examination demonstrated that the gingiva had recovered better in those from the main group comparing with the control group and manifested decrease of the gingival swelling, pain, bleeding and that was proved by changes of hygiene and periodontal indexes in comparison with the initial data. The positive dynamics was also observed in changes of hygiene and periodontal indexes and the patients from the main group exhibited more significant modifications. For example, OHI-S index and Fedorova-Volodkina indexes were 5,2 times lower comparing to the initial statistic (р<0,001) and 1,90 times (р<0,01) accordingly that indicates the good oral hygiene at this stage of the treatment. The assessment of the periodontal inflammation in the patients from the main group has indicated that after three month of the administered measures the Iodine number by Svrakov and PMA-index have decreased by 3,20 times (р<0,001) and 5,70 times (р<0,001) accordingly, the bleeding index has declined by 2,00 times (р<0,05). This facts proved the significant lessen of periodontal inflammation among these patients. The depth of periodontal pockets in the patients of main group has modified by 59,70% (р<0,05) that also suggested the decrease of swelling and gingival enlargement and elimination of the false pockets.

The analysis of oral hygiene and periodontal indexes within three month since the beginning of the treatment established the positive outcome in all patients despite the fact that in the main group the results proved to be better due to the complex therapy prescribed. The administration of immune-correctors in complex treatment of chronic hypertrophic gingivitis is efficient as it improves the results even in the early days after their introduction. Because of the fact that the investigated exponents were still abnormal, it is essential to appoint the general treatment of chronic hypertrophic gingivitis in orthodontic patients handled with bracket technique.
