THE FUNCTIONAL CONDITION OF MICROCIRCULATION IN PATIENTS WITH GENERALIZED PERIODONTITIS ON THE BACKGROUND OF TYPE I DIABETES MELLITUS WITH CARDIOMYOPATHY

Abstract

Diabetes mellitus is an important medical and social problem of humanity being in connection with the high prevalence and chronic course of the disease. It should be noted that according to the data by many literary sources diabetes mellitus and generalized defeats of periodontal tissues related to the pathogenesis with changes of all vascular system, in particular such as cause defeat of the blood vessels of tooth-jaw apparatus, significantly influence on the development of periodontal pathology, doing it more progressive.

The purpose of the work was to study the functional condition of microcirculation of periodontal tissues in patients with generalized periodontitis (GP) on the background of type 1 diabetes mellitus with cardiomyopathy.

The material and research methods. 105 patients, among of which the comparative group amounted 43 somatically healthy people with GP of initial − III degree of severity were examined. The main group amounted 62 patients with generalized periodontitis on the background of type 1 diabetes mellitus with cardiomyopathy. The classification of periodontal diseases conducted with the classification of M. F. Danilevsky and updated by means of paraclinical indices. For establishment of the functional condition of periodontal vessels was determined by the resistance of capillaries of gums using a vacuum test Kulazhenko (1960) and index of peripheral circulation (PCI) using the ALP-2 apparatus. The obtained data were worked out statistically.

Results of the research and their discussion. As the result of test on functional resistance of capillaries was established that in patients with generalized periodontitis of initial − first degree on the background of type 1 diabetes with cardiomyopathy the obtained values were in 1.4 times less compared to the data in patients with GP without somatic diseases − 49.15±2.21 seconds in the frontal and 86.00±3.25 seconds in the lateral area of the jaw.

In patients with GP of II degree of severity of the main group the time of formation of hematomas decreased to 20.40±2.36 seconds, p<0.01 in the frontal and to 42.50±3.21 seconds in the lateral area of the jaw, p<0.05.

In patients with GP of III degree of severity of the main group the time of formation of hematomas in both areas was the lowest − 13.00±1.49 seconds, p<0.01 in the frontal and 35.20±3.18 seconds in the lateral area, p1<0.01.

The analysis of the numerical values of PCI showed that in patients of the main group with GP of initial − II

degree of severity, condition of peripheral circulation may be regarded, according to the criteria of the index as satisfactory, and at GP of III degree of severity as decompensated. In patients of the comparative groups at all degrees of severity of GP, PCI was interpreted as unsatisfactory.

Conclusions. Thus, as the result of the obtained data was found that the condition of circulation in periodontal tissues in patients with generalized periodontitis characterized by microcirculatory violations. However, in patients with GP on the background of type 1 diabetes mellitus with cardiomyopathy these processes showed up more pronounced changes, confirmed by reduction time of formation on the mucous membrane of the gums and decrease of index of peripheral circulation.
