CORRELATION BETWEEN PROOXIDANT-ANTIOXIDANT BALANCE AND CHRONIC GINGIVITIS IN CHILDREN WITH ATOPIC DISEASES

Abstract

The aim of this study was to investigate the status and dependence of prooxidant-antioxidant balance in children with chronic gingivitis and without clinical signs of gingivitis on a background of atopic diseases.

Materials and methods. A total of 90 patients were examined at the Regional Children's Hospital №1 and the University Dental Center of Kharkov National Medical University. At the Regional Children's Hospital №1 60 children were examined who had been diagnosed "asthma," "allergic rhinitis", "atopic dermatitis." This contingent was a group of children of with atopic diseases. Based on the University Dental Center a group of healthy children without established somatic pathology was examined. For this study was selected 30 patients without clinical signs of dystrophic-inflammatory and inflammatory periodontal diseases with atopic diseases (group 1); 30 patients with chronic gingivitis on a background of atopic disease (group 2); as conventional physiological norm clinical and laboratory parameters of 30 patients with intact periodontal tissues without comorbidity were chosen (Group 3). Hygienic index for Fedorov has been received in all groups and did not exceed 1.2. Traditional procedures in all groups were identical and included the prevention of common diseases, eliminating bad habits, learning efficient oral hygiene.

Results. Analysis of the pro- and antioxidant homeostasis oral fluid of healthy patients without comorbidity (Group 3) and patients with clinically intact periodontal tissues, which diagnosed atopic diseases (group 1), showed that patients in the first group in the present study had growth of concentration of lipid peroxidation products. Thus, the concentration of MDA in oral fluid of healthy patients group without atopic diseases was lower by 1.6 times for this group of patients with clinically intact periodontal tissues, which diagnosed atopic diseases - (3,67±0,98) mmol / l (5,89±0,76) mmol / l, respectively. In patients of second group initial values of the concentration of free radical oxidation products were: MDA- (6,71±0,79) mmol / L, which is almost similar to the values in the first group, 1.8 times higher than in control. In patients without clinical manifestations of gingivitis, but with atopic diseases, also a significant decrease in the activity of non-enzymatic antioxidant system was noted. The level of glutathione was 1.8 times lower than in the second group than in the control, and was (2,71±0,61) mmol / l (4,98±0,57) mmol / L, respectively. In the first group patients level of glutathione was (2,23±0,54) mmol / L, which is 2.2 times lower than in the group of children without somatic pathology.

In the first and second groups of patients with clinically intact periodontal tissues and the presence of gingivitis the prooxidant-antioxidant balance were impaired in saliva towards the activation of free radical oxidation. It is especially noteworthy the fact that, given the growing concentration of MDA is not an adequate response from the enzymatic and non-enzymatic antioxidant defense units, indicating a loss of reliability of this system.

Conclusions. The study demonstrates changes in the state of prooxidant-antioxidant balance in children with atopic diseases towards the activation of free radical oxidation, which creates favorable conditions for the development of pathological changes in periodontal tissues. It should be noted that the presence of clinical signs of gingivitis is not critical to the severity of the imbalance prooxidant-antioxidant system of children with atopic diseases.
